Directions To Our Office Our Libertyville location is located at 1199 S Milwaukee Ave. You'll find our building situated in Greentree Plaza behind Bakers Square. Coming south on Milwaukee Ave, you can enter Greentree Plaza by the big sign. Go left and you'll find our building just down from XSport Fitness. Coming north on Milwaukee Ave, your best route is to take a left on Greentree Parkway. Then, an immediate right into the plaza parking lot where you'll find Beltone on your left side.
